CCHC Clinics

CCHC provides medical clinic services to all ages.

Elgibility
The Health Care Application is necessary for Children and Community Health Center of McKinney to confirm your eligibility for services provided by the clinic. Failure to supply the requested information may prolong the process. The information contained in this application may be verified to determine the eligibility of you and/or your family for free healthcare services. This application will be retained in our offices and periodically re-verified.

Along with this application, we will require the following documents:
· A picture ID (Driver’s License, Work photo ID, Mexico Voter Electoral Card, etc.)
· A copy of utility bill or rent statement with name and address

Additionally, we will require at least one of the following items with your application:
· Proof of current source of income. (Copies of the last three paycheck stubs or employer letter stating income and dates of service).
· A copy of statements or checks showing any other income received including disability, social security, unemployment pay, child support etc.
· A copy of any denial letter from Medicaid or other assistance program.
· A copy of your previous year’s Federal Income Tax Return.
· A letter stating how you are able to meet your daily living expense if you have no verifiable income

CCHC ACCEPTS ALL RESIDENTS REGARDLESS OF CITIZENSHIP STATUS. WE DO NOT ACCEPT ANY GOVERNMENT FUNDING AND THEREFORE ARE NOT REQUIRED TO DISCLOSE PERSONAL INFORMATION TO ANY STATE OR FEDERAL GOVERNMENT AGENCY.
